FORMS AND APPLICATIONS
The LWGCD has several different forms.  Please looks closely at the forms to determine which form is the
correct one you need and the requirements for that form.  If you are not sure, call the LWGCD office at (325)
728-2027.  All documents are PDF.

Well Permit Application- Used for those who want to drill a new well or replace an existing well.
   Requirements-
   1.  A $100.00 deposit must be placed before the District will consider the application.  The Deposit will be
        returned once the well has passed inspection by the District and all other requirements have passed
        (i.e. Plugging old wells that have been replaced, all wells being registered with the District).
   2.  District Personnel will preformed a site inspection before drilling and inspect the well after a Driller's
        Permit has been turned in, to ensure that all District and State Rules have been met.
   3.  All new wells must meet spacing requirements set forth by the District, unless an Well Easement
        Form is turned in with the application.  District Spacing requirements can be found in the District
        Rules.
   4.  You need to fill out as much of the application as possible.  
YOU ARE REQUIRED TO FILL OUT THE
         GALLONS PER MINUTE YOU EXPECT TO MAKE OR ARE REQUESTING.  WELL SPACING IS
         BASED OFF OF GPMS.

Well Easement- If you wish to drill a well closer to the property line than is allowable, this form must be
filled out and signed by the neighboring landowner that is to close and presented along with a Well Permit
Application.
   
Considerations
   The District will only grant variances of up to 25% from property lines and other wells.
   For example, District Rules state that you must be 400 ft. from the neighboring property line, however
   where you need to drill for a good wells is 275 ft from the neighbors property line.  If approved the
   District will grant you up to 25%, equaling 100 ft. in this example.  100 ft off of the required 400 ft leaves
   you with the new spacing of 300 ft.  You must be 300 ft. off of the neighboring property line.  In this
   synopsis, you are still to close and would need to find some other place to drill the well.

Change of Ownership- If you have purchased property and there are wells already in existence and
registered with the District, and you just need to change the ownership information

Well Registration-  For wells that are already in existence but are not registered with the district.  
   ***IMPORTANT***
   DO NOT USE IF YOU HAVE DRILLED A WELL WITHOUT PRIOR PERMISSION FROM THE DISTRICT.
